
Medical management is becoming a popular treatment option of unruptured ectopic pregnancy especially in women who wish to preserve their fertility and met the requisite criteria for this method. Successes have been reported even in those with higher Beta Human Chorionic Gonadotropin (?-HCG) levels contrary to prescribed guidelines and protocols.
We describe the case of a 35-year-old nulliparous woman with unruptured ectopic pregnancy and high ?-HCG level, who was managed successfully with a single-dose Methotrexate (MTX) treatment with good outcome. With close monitoring using serial ?-HCG and transvaginal ultrasound scanning, complete resolution of the pregnancy with remission of ?-HCG titre was successfully achieved thereby preventing surgical intervention and its complications.
The case highlights the importance of patient-centered care involving detailed and informed counselling, close follow-up, and multidisciplinary management of unruptured ectopic pregnancy.
